Caring Most About What Matters To You

Aging Friendly Advisory Services

🌿 Our Care Journey

Every family’s path is unique. At ElderCARE, we guide you through four key steps that define our professional eldercare services—always honoring your goals, values, and what matters most.

Assessment

All journeys begin with understanding where we’ve been and where we are going. Our comprehensive elder care assessment is the starting point, helping us uncover needs in health, home, and family life shaped by the priorities that matter most to you.

Who We Are — The ElderCARE Team

At ElderCARE, we know that aging can feel overwhelming for both older adults and the people who care for them. We’re here to walk beside you, offering the kind of professional ElderCARE services and guidance you’d want if it were your own family, in fact we often call ourselves “Professional Daughters”. Whether you’re planning or dealing with a more urgent need, we help bring clarity, support, and peace of mind.

Our team includes nurses, social workers, and therapy professionals who combine skills with compassion. Many of us hold ALCA certification, which reflects the highest standards in care management. But what matters most is simple: we care about you, and we’re committed to helping you find the right elder care solutions for your family.

We believe each person is a whole being - spiritual, physical, and emotional. That’s why we don’t just manage care; we listen, advocate, and support you every step of the way. Covering all middle Tennessee, we’re here to help you and your loved ones live with health, dignity, and independence.

Our Services

  • Care Management

At its heart, care management is about seeing the whole person, not just a diagnosis. It’s a way of pulling together all the pieces—medical, emotional, and practical—so that older adults and their families feel supported and less overwhelmed.

  • Aging Solo

Not everyone has a spouse, children, or close relatives nearby to lean on as they age. If you’re aging solo, you may worry about who will help with medical appointments, emergencies, or everyday decisions. At ElderCARE, our professional care managers step in as trusted guides and advocates—so you don’t have to face this journey alone.

  • Health Care Power of Attorney

One of the greatest concerns families and solo agers share is: “Who will speak for me if I can’t speak for myself?” At ElderCARE, our team of professional care managers can serve as your designated Health Care Power of Attorney (HCPOA). We make sure your wishes are honored, your plan is in place, and you are never alone in making healthcare decisions.

  • Navigating Long-Term Care Insurance

Long-term care insurance (LTCi) can be a lifeline for families, but understanding when and how to use it can feel confusing and frustrating. Policies are complex, claims are often delayed or denied, and many families simply don’t know where to start.

  • EAP Services for Employee/Employers

At ElderCARE, we partner with employers to provide education, resources, and hands-on support. Think of us as the “easy button” for employees navigating caregiving, so they can stay focused, engaged, and productive at work.

  • ElderCARE at HOME – Aging in Place

For many older adults, the greatest wish is simple: “I want to stay in my own home.” Aging in place allows individuals to remain in familiar surroundings, close to neighbors, friends, and community. But doing so safely and successfully often requires thoughtful planning and the right support.
