Two Service Tracks

Home Safety Assessment (Immediate Risk Focus)

Goal: Identify and reduce risks such as falls, tripping hazards, or unsafe bathroom setups.

Includes: Review of lighting, flooring, pathways, stairs, thresholds, grab bars, and emergency access.

Best for: Individuals at risk of falling, recovering from an injury, or advised to take preventive safety steps.

Note: If you are under a physician-ordered OT plan of care through home health, Medicare may sometimes cover this type of assessment. In that case, ElderCARE can partner with your home health company as a consultant to support their evaluation.

Home Livability & Aging-in-Place Assessment (Future Planning)

Goal: Ensure the home supports independence and comfort over time.

Includes: Recommendations for accessibility (ramps, wider doorways, turning space), adaptive equipment, kitchen/bathroom ease of use, and universal design solutions.

Best for: Adults planning to remain at home long-term, or those managing progressive conditions.

Note: This service is typically private-pay, since Medicare does not usually cover proactive or convenience-based planning.

  • Peace of Mind: Tools that support independence while keeping families connected and reassured.

Transparency & Payment

Many of our services are private-pay and not typically covered by Medicare.

  • If you are a Medicare beneficiary, we’ll explain clearly which portions may qualify under a therapy plan of care, and which are private-pay.

  • You’ll always receive full transparency about costs before we begin — no surprises.
