A Simple Way to Support Aging at Home

The 4Ms are:

What Matters

Good care starts by asking, “What matters most to you?”

A person may want to remain at home, attend church, spend time with family or avoid another hospital stay. ElderCARE listens to these goals and helps create a plan to support them.

Medication

Many older adults take several medications. Some may cause dizziness, confusion, weakness or appetite changes.

ElderCARE helps families keep medication information organized, prepare questions for medical visits and share concerns with doctors and pharmacists.

Mentation

Mentation means memory, thinking and mood.

Changes may include missed appointments, confusion, sadness, anxiety or trouble managing daily tasks. ElderCARE helps families notice these changes and connect with the right healthcare and community resources.

Mobility

Mobility means being able to move safely and stay active.

ElderCARE can coordinate physical therapy, equipment and other services. ElderCARE at HOME provides occupational therapy-led home assessments to identify fall risks and make daily activities easier.

Recommendations may include better lighting, grab bars, safer room layouts, helpful equipment or home technology.

Bringing the 4Ms Together

The 4Ms work best when used together.

For example, an older adult may want to remain at home. To support that goal, we may need to review medication concerns, watch for memory changes and improve safety throughout the home.
